Grace Requires Understanding’s adult day habilitation centers provide experiences that enhance the quality of life for adults with developmental disabilities who are on the D.D. waiver. We offer a fun, safe and friendly environment, where the individuals are free to make choices and are treated with dignity and respect.

The energetic adult day habilitation direct care staff assists individuals with developing and/or strengthening individualized skills, help to build relationships in the community, and assist in planning and implementing individualized specific goals. Both the individuals and the staff together plan outings to local attractions and into the community regularly.

GRU adult day habilitation centers tailor activities to each individual’s ISP goals and meaningful day objectives. Each center offers each individual a variety of activities to choose from each day. Each center maintains handicap capable vans to transport the individuals into the community each day. In addition, each center has a variety of indoor activities at the center in the event of inclement weather and/or for those individuals who choose these activities.

Adult Day Habilitation Services:

Daily functional and purposeful activities
Choice making throughout the day
Community membership
Implementation of ISP based upon each individual choice and skill building
Written assessments of the individual
Risk mitigation strategies with reasonable assurance of health and safety
Exploration of community inclusive work options
Career counseling
Vocational assessments
Age appropriate adult education
Opportunities to pursue age appropriate hobbies, recreation/leisure, etc.
Development of self-advocacy skills
Transportation
Personal care and daily living activities
Medication supports
Nursing and medical oversight services as needed
Integration of therapy plans including allowing therapist's access to facility
Development of social roles
Active engagement in community sponsored activities
Support needed to make to make measurable progress toward ISP's
Exploration and experience opportunities both independently and with others with like interests
Support of special diets

Adult Day Habilitation Staff Member Requirements:

Individual schedules
Supervision/staff support of individual
Effectively and respectively communicate with clients and each other
Contribute ideas to support ISP's
Assist and help accomplish goals
Model appropriate behaviors

Maintain safety standards and required documentation
Understand and implement crisis or emergency responses as needed
Report incidents
Maintain confidentiality
Implement DDSD standards, regulations, and performance contract requirements
